Whether pain, instability, and a decrease in movement or activity result from foot or ankle injuries or conditions resulting from arthritis, deformity, and other disorders, surgery typically becomes an option when issues are severe or non-surgical approaches have failed to reduce pain and restore function.

Surgical approaches may vary depending on the part of the ankle or foot affected, the severity of a condition, and other factors. However, they generally involve the repair of bones or soft tissues, including tendons and ligaments, which must be stabilized, reconstructed, or replaced. Some surgeries may be performed at one of our outpatient surgery centers, or require brief hospitalization in state-of-the-art operating rooms at Hoag Orthopedic Institute in Irvine. These can include:

• Arthroscopic Surgery – Arthroscopy is a minimally invasive surgical technique that allows surgeons to treat a number of painful foot and ankle conditions, including fractures, tears, tendonitis, and issues resulting from arthritis and past injuries. During the procedure, a small arthroscope (camera) is used to guide medical instruments inserted through tiny incisions in the foot or ankle, allowing surgeons to repair or reconstruct bones, ligaments, and other musculoskeletal structures. Because these incisions are much smaller than those used in standard open surgeries, patients may benefit from faster recoveries and less pain.
• Ligament Reconstruction – When ligaments of the foot or ankle are injured and cause instability, reconstruction surgery may be required. Ligament reconstruction may consist of repairing ligaments that have been stretched or torn by sewing them together or reattaching them to the bone with stiches. It may also involve the use of a tendon to replace a damaged ligament, reinforce the ankle, and provide stability. Common ligament procedures include lateral ligament repair, peroneus brevis tendon rerouting, peroneus brevis tendon loop, and peroneus brevis tendon split and rerouting.
• Ankle Replacement Surgery – Total ankle replacement surgery, or total ankle arthroplasty, is a procedure commonly used to treat chronic pain and decreased function as a result of arthritis, often as an alternative to arthrodesis (ankle fusion). It involves replacing structures of the ankle through incisions made in the front or side of the ankle with medical grade metal and plastic components that recreate the joint, sometimes in addition to lengthening of the Achilles tendon.
